目的探讨脑干动静脉畸形(AVM)容易出血的特有血管影像学表现及其原因。方法从血管解剖学和血流动力学的角度对14例脑干AVM的临床症状和血管影像学表现进行对照分析。结果14例脑干AVM中8例出血,占57.1%。2例直窦闭塞,其余12例中9例以单上Galen静脉(大脑大静脉)引流,占75%,9例中6例出血,占66.7%。8例A—V循环时间<2S,占57.1%,8例中5例出血,占62.5%。结论脑干AVM供血动脉压力高,血流速度快,A—V循环时间较短,且多以单支Galen静脉引流,相对引流不畅是脑干AVM容易出血的特有原因。
Objective To investigate the manifestations and causes of hemorrhage-specific vascular imaging of brainstem arteriovenous malformations (AVM). Methods From the perspective of vascular anatomy and hemodynamics, 14 cases of brainstem AVM clinical symptoms and vascular imaging were analyzed. Results In 14 cases of brainstem AVM, 8 cases were bleeding, accounting for 57.1%. 2 cases of straight sinus occlusion, the remaining 12 cases in 9 cases with single Galen vein (cerebral vein) drainage, accounting for 75%, 9 cases, 6 cases of bleeding, accounting for 66.7%. 8 cases of A-V cycle time <2S, accounting for 57.1%, 8 cases, 5 cases of bleeding, accounting for 62.5%. Conclusions The brainstem AVM feeding arteries have high pressure, fast blood flow, shorter A-V circulation time, and more single venous Galen venous drainage, and the relative poor drainage is the specific cause of AVM bleeding in brainstem.
