目的本研究应用瞬时波强(wave intensity,WI)技术了解心脏及血管功能的状态,评价其在研究2型糖尿病患者早期左室舒张功能中的使用价值。方法参照1999年WHO标准,根据血糖结果将门诊病人分为正常组(A组)、糖尿病前期组(B组)、糖尿病组(C组),均测量颈总动脉瞬时减速度波强(W2)、二尖瓣血流舒张早期峰值速度(E)和舒张晚期峰值速度(A)及其比值(E/A)、二尖瓣瓣环舒张早期峰值速度(Em)和舒张晚期峰值速度(Am)及其比值(Em/Am),并进行方差分析及各参数间的相关性分析。结果C组与A组比较,各参数差异显著(P<0.05);B组与A组比较,Em、Em/Am、Sm差异显著(P<0.05)。E/Em、E/A与Em/Am在各组中均显著分别呈负相关和正相关;W2与Em/Am仅在糖代谢异常组中显著呈负相关。结论 E/Em、Em/Am、E/A能较好地反映左室舒张功能,但对左室松弛功能的评价仍有局限性,W2评价左室舒张功能值得做更进一步的研究。
Purpose This study used the wave intensity (WI) technique to understand the status of cardiac and vascular function and evaluate its value in the study of early left ventricular diastolic function in patients with type 2 diabetes mellitus. Methods According to the 1999 WHO standard, outpatients were divided into normal group (group A), pre-diabetes group (group B) and diabetic group (group C) according to their blood glucose results. The mean carotid artery instantaneous deceleration intensity (W2) , Peak early mitral diastolic velocity (E) and peak diastolic velocity (A) and its ratio (E / A), mitral annular early diastolic peak (Em) and late diastolic peak velocity (Am) And its ratio (Em / Am), and analysis of variance and the correlation between the parameters. Results There was significant difference of each parameter between group C and group A (P <0.05). The difference of Em, Em / Am and Sm between group B and group A was significant (P <0.05). E / Em, E / A and Em / Am were significantly negatively correlated and positively correlated in each group; W2 and Em / Am were significantly negatively correlated only in abnormal glucose metabolism group. Conclusion E / Em, Em / Am and E / A can reflect the left ventricular diastolic function well, but the evaluation of left ventricular relaxation function is still limited. W2 evaluation of left ventricular diastolic function worth further study.
