1例81岁女性患者,因下颌骨左侧骨化纤维瘤伴面颊部蜂窝组织炎,使用头孢他啶3.0g加入5%葡萄糖注射液250mL,2次/d静脉滴注;0.5%奥硝唑100mL,1次/d静脉滴注。当晚患者出现不自主的头部及右手颤抖。实验室和心电图均未见明显异常;头颅CT检查,除轻度脑萎缩外无其他异常。给予地西泮10mg肌内注射;口服多巴丝肼片0.125mg,3次/d。3d后患者症状未见缓解。考虑震颤为头孢他啶引起的锥体外系反应。遂停用头孢他啶,改用头孢唑啉,其他用药不变。患者震颤症状消失,左侧面颊部蜂窝组织炎明显消退。
One 81-year-old female patient was treated with 3.0 g of ceftazidime and 250 mL of 5% dextrose injection twice daily with ceftazidime due to osteomyelofibroma on the left side of the mandible with 100 mL 0.5% 1 / d intravenous infusion. The patient showed involuntary head and right hand tremor on that night. Laboratory and ECG were no significant abnormalities; head CT examination, except for mild brain atrophy and no other abnormalities. Give diazepam 10mg intramuscularly; oral dopas hydrazine tablets 0.125mg, 3 times / d. Symptoms were not alleviated after 3 days. Consider tremor for ceftazidime-induced extrapyramidal reactions. Ceftazidime then disabled, switch to cefazolin, other medication unchanged. Patient symptoms disappeared tremor, left cheek cellulitis significantly subsided.
