目的探讨门诊输液患儿应用静脉留置针的临床风险及保障护理安全的方法。方法 286例门诊输液应用静脉留置针患儿,随机分为对照组和观察组,各143例。对照组进行常规穿刺,观察组增加预见性护理。分析留置针应用的临床风险,制定安全管理措施。对比两组患儿一次性穿刺成功率、留置时间、置管后并发症、护理满意度指标。结果观察组一次性穿刺成功率为97.9%(140/143),高于对照组的85.3%(122/143),差异具有统计学意义(χ~2=14.74,P<0.05)。观察组留置时间为(4.16±0.56)d,长于对照组的(1.42±0.13)d,差异具有统计学意义(t=56.99,P<0.05)。观察组静脉炎、液体渗漏、套管脱落等并发症发生率为3.5%(5/143),低于对照组的42.7%(61/143),差异具有统计学意义(χ~2=61.77,P<0.05)。观察组输液过程、总体效果、操作技能护理满意度均高于对照组,差异均具有统计学意义(t=29.60、23.09、48.16,P<0.05)。结论护士须提高安全意识,加强静脉留置针的护理安全管理,才能最大限度地发挥留置针的优点,确保临床护理安全。
Objective To investigate the clinical risk of intravenous catheter in pediatric patients with transfusion and the method of ensuring nursing safety. Methods 286 cases of outpatient infusion of intravenous indwelling needle children were randomly divided into control group and observation group, 143 cases each. Control group for routine puncture, observation group increased predictive care. Analyze the clinical risks of indwelling needles and develop safety management measures. The success rate of one-time puncture, indwelling time, complications after catheterization and nursing satisfaction index were compared between the two groups. Results The success rate of one-time puncture in the observation group was 97.9% (140/143), which was higher than that in the control group (85.3%, 122/143). The difference was statistically significant (χ ~ 2 = 14.74, P <0.05). The retention time of the observation group was (4.16 ± 0.56) d, which was longer than that of the control group (1.42 ± 0.13) d, the difference was statistically significant (t = 56.99, P <0.05). The incidence of complications such as phlebitis, fluid leakage and casing shedding was 3.5% (5/143) in observation group and 42.7% (61/143) in control group, with statistical significance (χ ~ 2 = 61.77 , P <0.05). The observation group infusion process, the overall effect, nursing skills satisfaction were higher than the control group, the differences were statistically significant (t = 29.60,23.09,48.16, P <0.05). Conclusion Nurses should raise awareness of safety and strengthen nursing safety management of intravenous indwelling needles to maximize the advantages of indwelling needles and ensure the safety of clinical nursing.
