目的探讨宁夏地区多囊卵巢综合征(PCOS)患者高雄激素血症对脂代谢的影响。方法收集PCOS患者267例,其中回族89例,汉族178例,分析高雄激素临床特征。根据血清睾酮水平,将PCOS患者分为高雄激素组和雄激素正常组,比较不同雄激素状态的脂代谢情况。结果①PCOS患者总体、回族、汉族痤疮发生率分别为20.60%、16.85%、22.47%。多毛发生率为25.09%、21.35%、26.97%;高雄激素血症发生率为47.56%、50.56%、46.06%。汉族多毛发生率高于回族(P<0.01)。②在总体PCOS患者中,高雄激素组总胆固醇(CHO)、载脂蛋白B(ApoB)高于雄激素正常组(P<0.05);在汉族PCOS患者中,高雄激素组CHO、低密度脂蛋白(LDL)、ApoB高于雄激素正常组(P<0.05或P<0.01)。在回族PCOS患者中,两组各项指标比较差异无统计学意义(P>0.05)。③在IR正常的非肥胖汉族PCOS患者中,高雄激素组CHO、ApoB高于雄激素正常组(P<0.05)。结论独立于肥胖、胰岛素抵抗因素,高雄激素血症是PCOS患者脂代谢紊乱的危险因素,且回、汉族间存在差异。
Objective To investigate the effects of androgens on the lipid metabolism in patients with polycystic ovary syndrome (PCOS) in Ningxia. Methods A total of 267 PCOS patients were collected, including 89 Hui patients and 178 Han patients. The clinical characteristics of Kaohsiung Hormone were analyzed. According to the level of serum testosterone, PCOS patients were divided into high androgen group and normal group of androgen to compare different androgen status of lipid metabolism. Results ① The incidence of acne in PCOS patients, Hui people and Han people were 20.60%, 16.85% and 22.47% respectively. The incidence of hirsutism was 25.09%, 21.35% and 26.97% respectively. The incidence of Holstein and Hormone was 47.56%, 50.56% and 46.06% respectively. Han hairy rate was higher than Hui (P <0.01). ② In total PCOS patients, the levels of total cholesterol (CHO) and apolipoprotein B (ApoB) were higher in the androgen group than in the normal androgen group (P <0.05); In the Han PCOS patients, the levels of CHO and low density lipoprotein ), ApoB was higher than that of normal androgen group (P <0.05 or P <0.01). In Hui PCOS patients, there was no significant difference between the two groups (P> 0.05). ③ In non-obese Han nationality PCOS patients with normal IR, the CHO and ApoB in the Kaohsiung hormone group were higher than those in the normal androgen group (P <0.05). Conclusion Independent of obesity, insulin resistance and hyperandrogenism are risk factors of lipid metabolism disorder in PCOS patients, and there are differences between the two groups.
