目的 对医疗单位正在使用的消毒液污染情况和紫外线灯管质量进行监测。方法 连续三年对某部12个驻京医疗单位进行监测。主要抽查换药室、手术室、口腔科、胃镜室等科室正在使用的消毒液和紫外线消毒灯管。消毒液检测含菌量超过250个/m1定为超标。EIA法检测HBsAg结果:样品OD值/阴性对照平均OD值≥2.1判为阳性。用SUY-3型紫外线照度订,在离紫外线灯管垂直照射1m处强度≥70mW/cm~2判定为合格。结果 使用中消毒液合格率(细菌指标)1994年为98.7%,1995年为94.1%,1996年为90.6%;HBsAg阳性率1994年为21.5%,1995年为2.9%,1996年为4.7%。紫外线灯管合格率呈逐年上升趋势,1994年69.1%,1995年78.8%,1996年为87.5%。结论 淘汰凉分低效消毒剂(如碳酸、新洁尔灭等)而改用高效消毒剂,或者将器械先用高压消毒方法处理后再置于低效消毒剂中保存,可降低HBsAg污染。使用过氧乙酸、戊二醛等消毒剂,应根据其降解速度和浸泡量及时更换。卫生队(所)紫外线灯管合格率均较门诊部、医院的低,主要原因是卫生人员消毒业务知识缺乏,紫外线灯管没有定期维护和监测。
Objective To monitor the disinfectant contamination and the quality of UV lamps being used by medical units. Methods Three 12-year medical units in Beijing were monitored for three consecutive years. The main sampling change dressing room, operating room, stomatology, endoscopy room and other departments are using disinfectant and UV disinfection lamp. Disinfectant test bacteria more than 250 / m1 set as excessive. EIA method to detect HBsAg results: Sample OD value / negative control average OD value of ≥ 2.1 sentenced as positive. SUY-3 type UV illuminance set, in the vertical irradiation from the UV lamp 1m intensity ≥ 70mW / cm ~ 2 was judged as qualified. Results The pass rate of disinfectant in use (bacterial index) was 98.7% in 1994, 94.1% in 1995 and 90.6% in 1996; the positive rate of HBsAg in 1994 was 21.5%, in 1995 it was 2.9% and in 1996 it was 4.7%. The pass rate of UV lamp shows a year-by-year upward trend, 69.1% in 1994, 78.8% in 1995 and 87.5% in 1996. Conclusion Elimination of cold sub-low-level disinfectant (such as carbonic acid, Bromozide, etc.) and the use of highly efficient disinfectant, or equipment first high-pressure sterilization method and then placed in low-efficient disinfectant to save, can reduce HBsAg contamination. The use of peracetic acid, glutaraldehyde and other disinfectants should be based on the speed of its degradation and timely replacement of the amount soaked. Health teams (by) UV pass rate than the clinic, the hospital is low, the main reason is the lack of knowledge of health personnel disinfection business, UV lamp is not regularly maintained and monitored.
