目的 观察肾血管平滑肌脂肪瘤 (RAML)组织形态学特点 ,探讨抗人类黑色素相关抗原 (HMB 4 5 )与RAML的相关性及诊断价值。方法 对 6 8例RAML进行组织形态学观察 ,采用免疫组化对其行HMB 4 5、抗黑色素 A (A10 3)、平滑肌肌动蛋白 (SMA)、肌特异性蛋白 (MSA)等多种抗体的检测 ,并收集其中 3例新鲜标本作电镜观察。结果 RAML由畸形血管、平滑肌细胞及脂肪混合组成 ,其中平滑肌细胞同时表达平滑肌标记和HMB 4 5。结论 RAML的病理形态变化多样 ,3种主要成分的比例和分布各异 ,在诊断和鉴别诊断中检测HMB 4 5是一个很有价值的指标。
Objective To observe the morphological characteristics of renal angiomyolipoma (RAML) and to investigate the correlation between the anti-human melanin-associated antigen (HMB 4 5) and RAML and the diagnostic value. Methods Histological observation was performed on 68 cases of RAML. Immunohistochemistry was used to detect the expression of RAMB in various tissues such as HMB 4 5, A10 3, SMA and MSA 3 cases of fresh specimens collected for electron microscopy. Results RAML consisted of a mixture of abnormal blood vessels, smooth muscle cells and fat, in which smooth muscle cells expressed both smooth muscle markers and HMB 4 5. Conclusion The pathological changes of RAML are diverse and the proportions and distribution of the three main components are different. Detection of HMB 4 5 in diagnosis and differential diagnosis is a valuable indicator.
