目的 系统评价磁共振成像对肺动脉栓塞的诊断价值.方法 计算机检索EMbase、PubMed、The Cochrane Library、CNKI和WanFang Data数据库,搜集磁共振诊断肺栓塞的相关文献,检索时限均从建库至2016年11月.由2名研究者按纳入和排除标准独立筛选文献、提取数据并评价纳入文献的偏倚风险,采用Stata12.0软件进行Meta分析.结果 共纳入14个研究,包含1 042例患者.Meta分析结果显示,磁共振对诊断肺栓塞的合并Sen、Spe、+LR、-LR、DOR分别为0.90 [95%CI (0.83,0.94)]、0.95 [95%CI (0.90,0.98)]、19.8 [95%CI (8.5,46.1)]、0.10 [95%CI (0.06,0.18)]、189 [95%CI (69,521)],SROC曲线下面积为0.97 [95%CI (0.95,0.98)].结论 磁共振对肺栓塞有良好的诊断价值,可作为疑诊肺栓塞患者的检查手段,特别是对CT肺动脉造影检查有禁忌证的患者.“,”Objective To systematic review the diagnostic value of magnetic resonance (MR) for pulmonary embolism (PE).Methods We electronically searched databases including EMbase,PubMed,The Cochrane Library,WanFang Data and CNKI from inception to November 2016,to collect the diagnostic studies about MR in the diagnosis of PE.Two reviewers independently screened literature according to the inclusion and exclusion criteria,extracted data,and assessed the risk of bias of included studies,and then meta-analysis was conducted using Stata 12.0 software.Results A total of 14 studies involving 1 042 patients were included.The pooled Sen,Spe,+LR,-LR and DOR were 0.90 (95%CI 0.83 to 0.94),0.95 (95%CI 0.90 to 0.98),19.8 (95%CI 8.5 to 46.1),0.10 (95%CI 0.06 to 0.18),189 (95%CI 69 to 521),respectively.The AUC of SROC were 0.97 (95%CI 0.95 to 0.98).Conclusion MR has a good diagnosis value for PE which can be regarded as an effective and feasible method for suspected PE patients,especially for those who has contraindication of computed tomographic pulmonary angiography.
