目的 对比分析3D双反转恢复(DIR)序列和颅脑MR常规扫描序列对脑转移瘤的检出率,探讨3D DIR序列检测脑转移瘤病灶的价值.方法 选择在本院经临床病理证实的肺癌患者28例,均行颅脑MR常规扫描、3D DIR、对比增强T1WI序列扫描,以对比增强T1WI检出的脑转移瘤数量为标准,由2名高年资医师共同分析判断颅脑MR常规扫描和3D DIR序列分别对脑转移瘤的检出率,对所得数据采用SPSS 19.0软件进行统计学分析,判断颅脑MR常规扫描、3D DIR对脑转移瘤的检出率有无统计学差异.结果 28例患者中,对比增强T1WI、颅脑MR常规扫描、3D DIR分别检出脑转移瘤病灶数量为91、62、79.采用配对四格表资料的x2检验,P<0.05,颅脑MR常规扫描、3D DIR对脑转移瘤的检出率有统计学差异.结论 3D DIR有效抑制了脑白质和脑脊液的信号,突出了脑转移瘤病灶的信号对比,在不使用对比剂的情况下对脑转移瘤有较高的检出率.“,”Objective To discuss the application of 3D double inversion recovery (DIR) sequence in detecting brain metastases by comparing the detection rate of brain metastases of DIR sequence with that of conventional MR scanning sequence.Methods A total of 28 patients with pathologically-confirmed lung cancers were enrolled in this study.Conventional MR imaging,3D DIR and contrast-enhanced T1 WI imaging were performed in all patients.Taking the lesion number detected by contrast-enhanced T1 WI images as standard,the detection rates of brain metastasis by conventional MRI and 3D DIR were evaluated by two experienced radiologists.The results were statistically analyzed using SPSS 19.0 software.Results In 28 patients,the metastatic lesions detected by contrast-enhanced T1WI,conventional MR and 3D DIR images were 91,62 and 79 respectively.Statistic analysis using pared data x2 test indicated that the difference in the detection rate of brain metastasis between conventional MRI and 3 D DIR was statistically significant (P < 0.05).Conclusion 3 D DIR technique can effectively suppress the signals of white matter and cerebrospinal fluid,thus the signals of brain metastasis lesions are highlighted and the detection rate of brain metastasis is improved in the situation of not using contrast agent.
