患者,男,58岁。发现左下颌角处包块5年多,于2000年8月来我院就诊。主诉:无任何症状。查体:血压120/75mmHg,心率75次/分,无颈静脉怒张,左下颌角处可见局部隆起,肿物大小约3.2cm×2.2cm,质地较硬,活动度差,有传导性搏动,压迫同侧颈总动脉后肿物不缩小,听诊无血管杂音。彩色多普勒超声显示左颈总动脉分叉处一中等偏低回声团块,边界尚清晰,但不规则。团块将颈内、颈外动脉明显向两边推移,并四面绕颈外动脉、三面围绕颈内动脉(图1、2)。还可见颈外动脉有分支至团块,团块内部可见较丰富的动、静脉血流信号。同侧颈内、外动脉管腔略有受压。提示:左颈总动脉分叉处实性
Patient, male, 58 years old. It was found that the mass at the left mandibular angle was more than 5 years old and came to our hospital in August 2000. Chief complaint: No symptoms. Physical examination: Blood pressure 120/75mmHg, heart rate 75 beats/min, no jugular vein engorgement, local bulge at the left mandibular angle, tumor size approximately 3.2cm x 2.2cm, hard texture, poor activity, conductive pulsation After the oppression of the common carotid artery, the tumor did not shrink and there was no auscultation. Color Doppler sonography showed a moderately hypoechoic mass at the bifurcation of the left common carotid artery. The boundary was still clear but irregular. The lumps push the internal and external carotid arteries distinctly to both sides, around the external carotid artery on all four sides, and the internal carotid artery on all three sides (Figs. 1 and 2). It can also be seen that the external carotid artery branches to the mass, and there are abundant signals of blood flow in the corps. The ipsilateral internal and external arteries were slightly compressed. Tip: The left common carotid artery bifurcation is solid
