目的评估三维彩色多普勒超声对慢性肾脏病(Chronic kidney disease,CKD)患者的诊断价值。方法研究共纳入海安县人民医院2014—2016年期间住院CKD患者共60例,根据病情不同分为1~5期。以同时期体检的30例健康人员作为对照组。使用三维彩色多普勒超声诊断仪对所有受检者叶间动脉阻力指数(RI)、血管指数(VI)、二维肾脏体积(2DRV)、三维肾脏体积(3DRV)和皮质厚度(CT)等指标进行检测,并进行各组间比较和分析。结果病例组5个不同分期RI水平低于对照组,VI和CT水平高于对照组(P<0.05);各组间RI、VI、2DRV、3DRV和CT水平不同,差异均有统计学意义(P<0.05)。CKD患者由1期至5期,RI水平逐渐降低,而VI、2DRV、3DRV和CT水平则逐步提高。结论三维彩色多普勒超声用于CKD诊断,无痛无创且成像效果好,值得临床推广应用。
Objective To evaluate the diagnostic value of three-dimensional color Doppler ultrasound in patients with chronic kidney disease (CKD). METHODS: A total of 60 hospitalized patients with CKD were enrolled in Hai’an County People’s Hospital from 2014 to 2016. They were divided into 1 to 5 stages according to their different conditions. The same period of physical examination of 30 healthy people as a control group. The RI, VI, 2DDRV, 3DRV and CT of all subjects were evaluated using three-dimensional color Doppler sonography. Indicators were tested and compared among groups and analyzed. Results The levels of RI in 5 different stages of the cases were lower than those in the control group, and the levels of VI and CT were higher than those in the control group (P <0.05). There were significant differences in RI, VI, 2DRV, 3DRV and CT among the groups P <0.05). From stage 1 to stage 5, the RI level gradually decreased in patients with CKD, while the levels of VI, 2DRV, 3DRV and CT were gradually increased. Conclusion Three-dimensional color Doppler ultrasound is used in the diagnosis of CKD. It is painless and non-invasive and has good imaging effect. It is worthy of clinical application.
