目的:探讨声辐射力脉冲弹性成像技术在儿童紫癜性肾炎(HSPN)肾脏损伤评估中的应用价值。方法:抽取2018年12月至2020年2月于郑州大学第一附属医院行肾脏穿刺活检并经病理证实的HSPN患儿50例作为HSPN组,抽取同期尿检正常的初发过敏性紫癜(HSP)患儿25例作为HSP组,另招募25例健康儿童作为对照组;并将HSPN组患儿按24 h尿蛋白量分为轻度组、中度组、重度组3个亚组。用声触诊弹性成像技术定量分析各组研究对象的肾皮质剪切波速度(SWV)值,并比较各组间SWV的差异。结果:HSPN组、HSP组、对照组的肾皮质SWV值分别为(2.68±0.16)、(2.36±0.07)、(2.53±0.07)m/s; HSPN组与HSP、对照组比较差异有统计学意义(n P0.05)。低度组、中度组、重度组SWV值分别为(2.71±0.15)、(2.72±0.07)、(2.71±0.11)m/s,HSPN组3个亚组间SWV值比较差异未见统计学意义(n P>0.05)。n 结论:声辐射力脉冲弹性成像技术可用于早期评估HSPN患儿的肾皮质弹性改变,但对HSP及不同尿蛋白水平的HSPN患儿的肾弹性的评估价值有限。“,”Objective:To investigate the value of acoustic radiation force imaging (ARFI) in the evaluation of the renal damage in children with henoch schonlein purpura nephritis (HSPN).Methods:A total of 50 children with HSPN confirmed by pathology in the First Affiliated Hospital of Zhengzhou University from December 2018 to February 2020 were selected as HSPN group; and 25 children with primary henoch schonlein purpura (HSP) and normal urine test were selected as HSP group; another 25 healthy children were recruited as control group. According to 24 h urine protein content, 50 children of the HSPN group were divided into three subgroups: mild group, moderate group, and severe group. The shear wave velocity (SWV) of renal cortex was quantitatively analyzed by acoustic touch elastography, and the SWV values were compared among these groups.Results:The SWV values of renal cortex in HSPN group, HSP group and control group were (2.68±0.16)m/s, (2.36±0.07)m/s and (2.53±0.07)m/s, respectively. The SWV value of HSPN group was significantly different from that of HSP group, and that of control group (n P0.05). The SWV values of mild group, moderate group, and severe group were (2.71±0.15)m/s, (2.72±0.07)m/s and (2.71±0.11)m/s, respectively; and there was no statistical difference among the three subgroups of HSPN group (n P>0.05).n Conclusions:ARFI can be used for early evaluation of renal cortex elasticity in children with HSPN, but it has limitation in evaluating the renal elasticity of children with HSP, and HSPN children with different urinary protein levels.
