目的观察静脉应用胺碘酮对电击伤并发心房颤动的疗效和安全性。方法 12例电击伤患者,心电图证实为心房颤动。在常规治疗的基础上予胺碘酮150 mg静脉注射,继之用微量泵以0.5~1.5mg/min维持泵入,持续24 h。观察心律转复情况、心室率、血压、QT间期及其不良反应。结果 12例患者均成功转复为窦性心律,转复成功率100%。平均转复时间为(285±75)min。心房纤颤转复为窦性心律前后心室率变化有统计学意义(P<0.01);动脉收缩压、舒张压差异无统计学意义(P>0.05);在用药后24 hQT间期变化有统计学意义(P<0.05)。无严重不良反应。结论静脉应用胺碘酮治疗电击伤并发的心房颤动是安全、有效的方法。
Objective To observe the efficacy and safety of intravenous amiodarone in the treatment of electric shock with atrial fibrillation. Methods 12 cases of electric shock patients, ECG confirmed as atrial fibrillation. On the basis of routine treatment of amiodarone 150 mg intravenously, followed by micro-pump at 0.5 ~ 1.5mg / min to maintain pumped for 24 h. Observation of heart rate recovery, ventricular rate, blood pressure, QT interval and adverse reactions. Results All 12 patients were successfully converted to sinus rhythm, the successful rate of 100%. The average recovery time was (285 ± 75) min. There were significant differences in ventricular rate before and after the atrial fibrillation was converted to sinus rhythm (P <0.01); there was no significant difference in arterial systolic pressure and diastolic pressure (P> 0.05); there was statistical significance in QT interval after 24 h Significance (P <0.05). No serious adverse reactions. Conclusion Intravenous amiodarone is safe and effective in the treatment of atrial fibrillation associated with electrical injury.
