目的观察自体细胞因子诱导杀伤(cytokine-induced killer,CIK)细胞联合全身化疗及胸腔内药物注射治疗恶性胸腔积液的近期疗效和不良反应。方法恶性胸腔积液患者56例均给予全身化疗+静脉滴注CIK细胞+胸腔内注射药物(干扰素1 000万u+白细胞介素-2 200万u+生理盐水50mL)。治疗42d后评价近期疗效、生活质量改善及不良反应发生情况。结果治疗42d后完全缓解26例,部分缓解23例,稳定7例,总有效率87.5%;生活质量好转45例,稳定9例,恶化2例,生活质量改善率80.4%;胸痛、发热、恶心、呕吐、骨髓抑制以Ⅰ~Ⅳ度为主,给予解热镇痛、止吐及升白细胞药物治疗后好转。结论 CIK细胞联合全身化疗及胸腔内药物注射可有效控制恶性胸腔积液,提高患者生存质量,患者耐受性好。
Objective To observe the short-term effects and side effects of cytokine-induced killer (CIK) cells combined with systemic chemotherapy and intrathoracic drug injection in the treatment of malignant pleural effusion. Methods 56 cases of malignant pleural effusion were given systemic chemotherapy + intravenous infusion of CIK cells + intrathoracic injection of drugs (interferon 10 million u + interleukin -2 million u + saline 50 mL). After 42 days, the curative effect, quality of life improvement and adverse reactions were evaluated. Results After treatment for 42 days, 26 cases were completely relieved, 23 cases partially relieved, 7 cases stable, the total effective rate was 87.5%, 45 cases were improved in quality of life, 9 cases were stable, 2 cases were deteriorated and the quality of life improved by 80.4%. Chest pain, fever, nausea , Vomiting, myelosuppression mainly Ⅰ ~ Ⅳ, to give antipyretic analgesic, antiemetic and leukocyte drug treatment improved. Conclusion CIK cells combined with systemic chemotherapy and intra-thoracic injection of drugs can effectively control malignant pleural effusion, improve patient quality of life, patients with good tolerance.
