我们在1979~1980年春将肺心病急发期病人入院后测定血气,凡PaCO_2超过50mmHg者即给以吸氧及一般治疗,其中有22例病情较重者同时加用间歇辅助通气作为治疗组;未用辅助通气者26例为对照组.治疗前PaCO_2在治疗与对照两组中分别为61.45±2.48mmHg及61.51±1.94mHg,前者有神志改变者11例,后者仅1例.治疗组在治疗后1~6天PaCO_2减低非常显著(p<0.01),而对照组均无显著性(p>0.05).治疗组16例测定pH,其中10例pH<7.35,治疗后除2例外,均升至正常.对照组仅3例测定pH,2例pH<7.35,治疗后1例更低.临床有神志改变者,治疗组11例,治疗的当日或第二日清醒者5倒,治疗后2~3天清醒者各1例,死亡3例.对照组1例第6天清醒,死亡1例.此法为即时纠正缺氧及二氧化碳潴留提供了一个简便、可靠的方法,且在基层单位以手挤式简易呼吸机也可取得同样疗效.
In the spring of 1979 ~ 1980, we determined the blood gas of patients with acute pulmonary heart disease admitted to hospital. All patients with PaCO_2 more than 50mmHg were given oxygen and general treatment. Among them, 22 cases were in severe condition with intermittent assisted ventilation as the treatment group. 26 cases without the use of auxiliary ventilation as the control group before treatment, PaCO_2 in the treatment and control groups were 61.45 ± 2.48mmHg and 61.51 ± 1.94mHg, the former have conscious change in 11 cases, the latter only in 1.
