目的:观察在西药治疗基础上,采用知柏地黄汤联合补中益气汤加减治疗老年性耳鸣的临床疗效。方法:选择94例肾精亏损兼脾胃虚弱的老年性耳鸣患者为研究对象,随机分为对照组和治疗组各47例。对照组给予盐酸氟桂利嗪胶囊内服治疗,治疗组则在西药内服的基础上给予由知柏地黄汤和补中益气汤化裁而成的中药内服治疗,2组均以1周为1疗程,共治疗2疗程。观察2组患者耳中鸣响、听力下降、失眠、乏力、焦虑烦躁等临床症状的改善情况,并对证候进行评分,观察评分的变化情况。结果:对照组总有效率为72.3%,低于治疗组的总有效率91.5%,2组比较,差异有统计学意义(P<0.05)。治疗后,2组耳中鸣响、听力下降、失眠、乏力、焦虑烦躁积分均较治疗前降低(P<0.05),治疗组各项证候积分均低于对照组(P<0.05)。结论:在西药治疗基础上加用知柏地黄汤与补中益气汤化裁而成的汤剂,能有效改善肾精亏损、脾胃虚弱型患者的耳鸣症状与听力,疗效确切。
OBJECTIVE: To observe the clinical effects of using Zhibo Di Huang Tang combined with Buzhong Yiqi Decoction in the treatment of senile tinnitus on the basis of Western medicine. Methods: Ninety-four cases of nephrotic tinnitus patients with deficiency of kidney essence and spleen-stomach were randomly divided into control group and treatment group (n = 47). The control group was treated with Flunarizine Hydrochloride Capsule orally, and the treatment group was treated with traditional Chinese medicine made by Zhibai Dihuang Decoction and Buzhongyiqi Tang, on the basis of Western medicine orally. Both groups were treated with 1 week as 1 Course of treatment, a total of 2 courses of treatment. The improvement of clinical symptoms such as ringing, hearing loss, insomnia, fatigue, anxiety and irritability in the ears of the two groups were observed and the syndromes were scored to observe the changes of the scores. Results: The total effective rate of the control group was 72.3%, which was lower than the total effective rate of the treatment group 91.5%. There was significant difference between the two groups (P <0.05). After treatment, the sounding, hearing loss, insomnia, fatigue and anxiety and anxiety scores of the two groups were lower than those before treatment (P <0.05). The scores of syndromes in the treatment group were lower than those in the control group (P <0.05). Conclusion: Based on the western medicine treatment, the decoction of Zhibo Dihuang Decoction and Buzhongyiqitang decoction can effectively improve the tinnitus symptoms and hearing of patients with deficiency of spleen and stomach and tinnitus, and the curative effect is definite.
