目的探讨胱抑素C(Cys C)与原发性高血压患者颈动脉内膜-中层厚度(IMT)的关系。方法选择138例原发性高血压患者,检测患者血清Cys C、生化及血压指标;应用彩色多普勒超声检查仪测定颈动脉IMT,按照颈动脉IMT将原发性高血压患者分为IMT正常组和IMT增厚组,对两组血清Cys C、生化、年龄及血压指标进行比较,并分析上述指标与颈动脉IMT的关系。结果原发性高血压患者IMT增厚组血清Cys C明显高于IMT正常组[(1.32±0.42)vs(1.11±0.28)mg/L](t=2.69,P<0.05)。Spearman相关分析发现颈动脉IMT与血清Cys C、年龄显著相关(r=0.398,P<0.01;r=0.512,P<0.01),多元逐步回归分析显示Cys C并非颈动脉IMT的独立危险因素;在调整年龄进行偏相关分析显示Cys C与IMT的相关性差异无统计学意义(r=0.047,P>0.05)。结论血清Cys C与原发性高血压颈动脉IMT无确切关联,不能作为其预测指标。
Objective To investigate the relationship between cystatin C (Cys C) and carotid artery intima - media thickness (IMT) in patients with essential hypertension. Methods One hundred and thirty-eight patients with essential hypertension were enrolled in this study. Serum Cys C, biochemical and blood pressure indices were measured. The carotid IMT was measured by color Doppler sonography, and the patients with essential hypertension were divided into normal IMT Group and IMT thickened group, the serum Cys C, biochemical, age and blood pressure indicators were compared, and the relationship between these indicators and carotid IMT. Results The serum Cys C of patients with IMT was significantly higher than that of patients with IMT (1.32 ± 0.42 vs 1.11 ± 0.28 mg / L, t = 2.69, P <0.05). Spearman correlation analysis showed that carotid IMT was significantly associated with serum Cys C and age (r = 0.398, P <0.01; r = 0.512, P <0.01). Multiple stepwise regression analysis showed that Cys C was not an independent risk factor for carotid IMT. Partial correlation analysis showed that the correlation between Cys C and IMT was not statistically significant (r = 0.047, P> 0.05). Conclusion Serum Cys C is not correlated with carotid IMT in patients with essential hypertension and can not be used as a predictor.
