目的观察食管胃交界神经内分泌肿瘤的病理形态。方法对216例食管胃交界肿瘤分别用Synapto-physin、Chromogranin、CD56和Neuron-specific enolase(NSE)四种抗体进行免疫组化检测,筛选出51例神经内分泌肿瘤,进行病理形态学观察。结果 51例神经内分泌肿瘤中包括单纯性神经内分泌癌9例,混合性外分泌—内分泌癌42例。Synaptophysin、Chromogranin、CD56和NSE四种抗体在单纯性神经内分泌癌中呈弥漫强阳性表达,而在混合性外分泌—内分泌癌中的神经内分泌癌区域呈不同强度表达。51例神经内分泌肿瘤中溃疡型39例、局部浸润型6例、弥漫浸润型4、黏膜局部隆起型2例,侵及网膜37例、浆膜层7例、肌层5例、黏膜下层2例,肿瘤长径2.0~7.0 cm。结论食管胃交界神经内分泌肿瘤病理形态比较复杂,需要依据免疫组化检查与其他肿瘤进行鉴别诊断。
Objective To observe the pathological morphology of neuroendocrine tumors at the esophagogastric junction. Methods Totally 216 cases of esophageal and gastric tumors were detected by immunohistochemistry with Synapto-physin, Chromogranin, CD56 and Neuron-specific enolase (NSE) antibodies respectively. Fifty-one neuroendocrine tumors were screened for histopathological observation. Results 51 cases of neuroendocrine tumors including simple neuroendocrine carcinoma in 9 cases, mixed exocrine - endocrine cancer in 42 cases. Synaptophysin, Chromogranin, CD56 and NSE antibodies were diffusely and strongly expressed in simple neuroendocrine carcinomas, while neuroendocrine carcinomas in mixed exocrine and endocrine cancers showed different intensity expression. 51 cases of neuroendocrine tumors in 39 cases of ulceration, local infiltration in 6 cases, diffuse infiltration 4, 2 cases of local mucosal uplift invaded 37 cases of omentum, serosa in 7 cases, 5 cases of myometrial, submucosa 2 For example, tumor diameter 2.0 ~ 7.0 cm. Conclusion The pathological morphology of neuroendocrine tumors at the junction of the esophagus and stomach is rather complicated and needs to be differentiated from other tumors by immunohistochemistry.
