目的评价窄带成像技术(NBI)的应用对结肠息肉的诊断价值。方法对2013年8月-2014年8月经天津市咸水沽医院结肠镜检出的69例结肠息肉患者使用窄带成像技术观察息肉表面血管形态(CP),对病理分型进行预测,将息肉切除后的标本进行病理学检查,研究表面血管形态与病理学的关系。结果 69例患者中有163处病变,依据病理诊断结果 ,共有93处肿瘤性病变,70处非肿瘤性病变,普通NBI观察结肠隆起性病变时,敏感性为93.50%,特异性为81.40%,准确度为88.34%。而直径<5mm的病变,敏感性为94.40%,特异性为96.00%,准确度为95.19%;同时,直径>5mm的病变,敏感性为92.30%,特异性为45.00%,准确度为76.27%。结论 NBI模式下观察结肠隆起性病变的CP形态对预测其病理分型,即鉴别其为肿瘤性或非肿瘤性病变具有较为可靠的价值。
Objective To evaluate the diagnostic value of narrowband imaging (NBI) for colonic polyps. Methods From June 2013 to August 2014, 69 cases of colon polyps detected by colonoscopy in Xianshuigu Hospital of Tianjin City were examined by narrow-band imaging to observe the surface morphology of the polyps (CP), and the pathological classification was predicted. The polyps were excised After the specimens for pathological examination, the study of surface vascular morphology and pathological relationship. Results There were 163 lesions in 69 patients. According to the pathological findings, there were 93 neoplastic lesions and 70 non-neoplastic lesions. The sensitivity, specificity and specificity of NBI were 93.50%, 81.40% The accuracy is 88.34%. While the diameter less than 5mm, the sensitivity was 94.40%, the specificity was 96.00%, the accuracy was 95.19%. Meanwhile, the diameter was> 5mm, the sensitivity was 92.30%, the specificity was 45.00% and the accuracy was 76.27% . Conclusions The observation of the morphological changes of colon in NBI under the NBI mode has a reliable value in predicting the pathological type of the colonic lesions, that is, identifying the tumor as a tumor or a non-neoplastic lesion.
