目的探讨慢性阻塞性肺疾病(COPD)患者诱导痰中白细胞介素-18(IL-18)的水平及其于炎性细胞数、肺功能的关系。方法分别选择46例COPD患者和17例健康对照组进行肺功能测定和诱导痰检查方法对痰炎性细胞进行分类计数,并用酶联免疫吸附试验法测定诱导痰上清液中IL-18、基质金属蛋白酶-9(MMP-9)和基质金属蛋白酶抑制剂-1(TIMP-1)浓度。结果 COPD急性加重期组诱导痰IL-18的水平与中性粒细胞数及巨噬细胞数呈正相关(r值分别为0.56、0.72,均P<0.05);与FEV1占预计值%、FEV1/FVC呈负相关(r值分别为-0.57、-0.61,均P<0.01);与MMP-9浓度呈正相关(r=0.67,P<0.01)。COPD急性加重期组诱导痰MMP-9水平与中性粒细胞数呈正相关(r=0.68,P<0.05)。COPD急性加重期组诱导痰MMP-9浓度与FEV1占预计值%呈负相关(r=-0.64,P<0.01)。COPD急性加重期组诱导痰中MMP-9/TIMP-1比值与与FEV1占预计值(呈正相关(r=0.61,P<0.01)。结论 COPD患者诱导痰中IL-18的水平与气道炎症和气流受限有关。
Objective To investigate the level of interleukin-18 (IL-18) in sputum induced by chronic obstructive pulmonary disease (COPD) and its relationship with inflammatory cell number and pulmonary function. Methods Forty-six patients with COPD and 17 healthy controls were selected for pulmonary function test and sputum test to classify and count sputum inflammatory cells. ELISA and ELISA were used to determine the levels of IL-18, Metalloproteinase-9 (MMP-9) and matrix metalloproteinase inhibitor-1 (TIMP-1) concentrations. Results The level of IL-18 in sputum induced by acute exacerbation of COPD was positively correlated with the number of neutrophils and macrophages (r = 0.56, 0.72, respectively, P <0.05). Compared with the predictive value of FEV1, FEV1 / FVC (r = -0.57, -0.61, respectively, P <0.01), but positively correlated with the concentration of MMP-9 (r = 0.67, P <0.01). The level of MMP-9 in induced sputum of COPD acute exacerbation group was positively correlated with neutrophil count (r = 0.68, P <0.05). The concentration of MMP-9 in sputum induced by acute exacerbation of COPD was negatively correlated with the predicted value of FEV1 (r = -0.64, P <0.01). The ratio of MMP-9 / TIMP-1 in induced sputum and the expected value of FEV1 in COPD acute exacerbation group was positively correlated (r = 0.61, P <0.01) .Conclusion The level of IL-18 in sputum induced by COPD is correlated with airway inflammation And limited airflow related.
