目的探讨声速匹配(SSC)技术与声触诊组织量化(VTQ)技术在评估移植肾纤维化程度中的临床价值及两者的相关性。方法选择接受移植肾穿刺活检或手术切除的患者95例,对移植肾行SSC和VTQ检查,病理结果依据BANFF2005中的标准将移植肾纤维化程度分为无纤维化、轻度纤维化、中度纤维化和重度纤维化共4组(G0~G3组),并对检查结果进行对比分析。结果移植肾无纤维化组至重度纤维化组的区域声速值(ZSS)依次增快,G0组与G1组、G 1组与G2组的ZSS值之间差异无统计学意义(P>0.05);G0组与G2组、G1组与G3组、G2组与G3组以及G0组与G3组的ZSS值比较差异均有统计学意义(P<0.05)。移植肾无纤维化组至重度纤维化组的VTQ值明显增快,G0组和G1组的VTQ值的差异无统计学意义(P>0.05),但G0组或G1组与G2组及G3组的VTQ值两两之间差异均有统计学意义(P<0.05)。ZSS值与VTQ值呈正相关(r=0.62 P<0.001),回归方程y=1 490.6x-23.9。结论声速匹配对评估移植肾纤维化有一定价值,但尚不能对移植肾纤维化程度进行精确分度。
Objective To investigate the clinical value of sonic velocity matching (SSC) and voice palpation tissue quantification (VTQ) in assessing the degree of renal allograft fibrosis and their relationship. Methods Ninety-five patients who underwent transplanted renal biopsy or surgical resection were examined by SSC and VTQ. The pathological results were divided into three groups according to the standard of BANFF2005: fibrosis, mild fibrosis, moderate fibrosis, Fibrosis and severe fibrosis, a total of 4 groups (G0 ~ G3 group), and comparative analysis of test results. Results There was no significant difference in ZSS between G0 group and G1 group, G 1 group and G2 group (P> 0.05) ; There were significant differences in ZSS between G0 group and G2 group, G1 group and G3 group, G2 group and G3 group, G0 group and G3 group (P <0.05). There was no significant difference in VTQ between G0 group and G1 group (P> 0.05), but there was no significant difference between G0 group and G1 group and G2 group and G3 group VTQ value between the two pairs of differences were statistically significant (P <0.05). There was a positive correlation between ZSS and VTQ (r = 0.62 P <0.001). The regression equation y = 1 490.6x-23.9. Conclusion Acoustic velocity matching is valuable in assessing renal allograft fibrosis, but it is not yet possible to classify the degree of renal graft fibrosis accurately.
