目的研究血友病A型患者疾病的严重程度与焦虑、抑郁情绪的关系,以及对患者生活质量的影响。方法将33例血友病A型患者按疾病严重程度分为重型组(FⅧ浓度<1%)8例和轻/中型组(FⅧ浓度≥1%~25%)25例,采用广泛性焦虑量表(GAD-7)、抑郁症筛查量表(PHQ-9)评估患者的焦虑和抑郁情绪,采用健康调查问卷(SF-36)评估患者的生活质量,比较不同严重程度患者焦虑抑郁的状态以及对生活质量的影响。结果重型组GAD-7及PHQ-9量表分值均高于轻/中型组,差异均有统计学意义(均P<0.05);SF-36量表中生理职能的分数两组间差异有统计学意义(P<0.05)。结论重型血友病A型患者的焦虑、抑郁程度显著高于轻/中型患者;重型血友病A型患者与轻/中型血友病A型患者之间生活质量的差异主要体现在生理职能的变化上。
Objective To study the relationship between severity of illness and anxiety and depression in patients with hemophilia A and the impact on quality of life of patients. Methods Thirty-three hemophilia A patients were divided into severe group (8 cases with FⅧ concentration <1%) and 25 cases with light / medium group (FⅧ concentration≥1% ~ 25%) according to the disease severity. (GAD-7) and Depression Rating Scale (PHQ-9) were used to evaluate the anxiety and depression of the patients. The health questionnaire (SF-36) was used to evaluate the quality of life of the patients and the anxiety and depression status of patients with different severity As well as the impact on quality of life. Results The scores of GAD-7 and PHQ-9 in the severe group were significantly higher than those in the mild-moderate group (all P <0.05). The scores of physiological function in the SF-36 scale were Statistical significance (P <0.05). Conclusions The severity of anxiety and depression in patients with severe hemophilia A are significantly higher than those in patients with mild / moderate hemophilia. The differences in quality of life between patients with severe hemophilia A and patients with mild to moderate hemophilia A are mainly manifested in physiological function Change.
