目的探讨超声弹性成像(UE)硬度评分法及应变率比值法在诊断乳腺占位性病变中的价值.方法对78例,共156个乳腺占位性病变术前行超声弹性成像检查.测量病变组织与周围组织弹性应变率比值,以3.08为界点判断占位性病变的良恶性.超声弹性成像硬度评分标准采用改良5分法,1-3分为良性,4-5分为恶性.以手术病理为金标准,对比分析超声弹性成像硬度评分法、应变率比值法以及二者联合应用在诊断乳腺占位性病变良恶性的准确性.结果156个占位性病变中,恶性15个,良性141个.超声弹性成像硬度评分法诊断乳腺良恶性病变的准确性91%,灵敏度99.3%,特异性86.7%.弹性应变率比值法诊断乳腺良恶性病变的准确性92.3%,灵敏度99.3%,特异性73.4%.联合应用超声弹性成像硬度评分法及弹性应变率比值法,诊断乳腺占位性病变良恶性的准确性98.1%,灵敏度98.6%,特异性93.3%.结论超声弹性成像硬度评分及弹性应变率比值在乳腺占位性病变良恶性诊断中均有较高价值,但两者结合可以提高其诊断准确性.“,”Objective To investigate the clinical value of ultrasound elastography (UE) score and the elastic strain rate ratio in the diagnosis of breast masses .Methods 78 patients with breast masses(156 masses) were examined with ultrasound elastography. The masses’ and its surrounding tissues’ elastic strain rate ratio were measured . 3.08 was regard as the boundary points to differentiate the masses to be benign or malignant. The elastogram was graded using 5-score evaluating method. 1-3 points were graded to be benign, and 4-5 points were graded to be malignant. Using the postoperative pathological diagnosis as gold standard to comparative analyse the diagnosis accuracy of ultrasound elastography score and the elastic strain rate ratio in differenting benign and malignant breast masses . Results Of the 156 masses, 15 were malignant and 141 were benign. Accuracy of elastography score in differentiating breast masses was 91%, the sensitivity was 99.3% and the specificity was 86.7%; Accuracy of elastic strain rate ratio in differentiating breast masses was 92.3%, the sensitivity was 99.3% and the specificity was 73.4%;Accuracy of elastography score combined with elastic strain rate ratio in differentiating breast masses was 98.1%, the sensitivity was 98.6% and the specificity was 93.3%. Conclusion Both the elastography score and elastic strain rate ratio was valuable ultrasonographic method in differntiating benign and malignant breast masses. And combining these two can improve the validity in the diagnosis of malignant breast tumors.
