目的:系统评价糖尿病病人添加富含n-3多不饱和脂肪酸(PUFA)的营养补充剂是否可以改善病人血糖和血脂的状况。方法:检索MEDLINE、EMBASE、Cochrane对照试验中心注册数据库(CENTRAL),检索语言为英语,检索起止时间为2000年至2014年。检索词为Diabetes,n-3 polyunsaturated fatty acid,fish oil,clinical trial。纳入对糖尿病病人添加n-3 PUFA的随机对照试验,采用Jadad量表进行研究质量评价,使用Rev Man 5.2软件进行Meta分析。结果:最终纳入6项来自5个国家的高质量研究数据资料。n-3 PUFA组病人血清瘦素水平低于对照组,且差异有统计学意义。随机血糖、糖化血红蛋白、胰岛素、三酰甘油、胆固醇、高密度脂蛋白、低密度脂蛋白和脂联素等,两组之间差异均无统计学意义。结论:现有临床证据不能证明膳食补充n-3 PUFA对改善糖尿病病人的血糖和血脂状况有益。
OBJECTIVE: To systematically evaluate whether the addition of nutritional supplements rich in n-3 polyunsaturated fatty acids (PUFA) to diabetic patients can improve the blood glucose and lipid profile of patients. Methods: MEDLINE, EMBASE and Cochrane Central Register of Controlled Trials (CENTRAL) were searched. The search language was English. The search period was from 2000 to 2014. The search term is Diabetes, n-3 polyunsaturated fatty acid, fish oil, clinical trial. A randomized controlled trial of n-3 PUFA in diabetic patients was included. The quality of the study was assessed using the Jadad scale and Meta-analysis was performed using Rev Man 5.2 software. Results: Finally, six high quality research data from five countries were included. The level of serum leptin in n-3 PUFA group was lower than that in control group, and the difference was statistically significant. Random blood glucose, glycosylated hemoglobin, insulin, triglyceride, cholesterol, high density lipoprotein, low density lipoprotein and adiponectin, etc. There was no significant difference between the two groups. CONCLUSION: The available clinical evidence does not demonstrate that dietary supplementation of n-3 PUFA is beneficial in improving glycemia and lipid profile in diabetic patients.
