目的探讨乳腺钼靶选择性乳腺导管造影结合影像报告与数据系统(BI-RADS)对诊断无肿块乳头溢液乳腺疾病的临床应用价值。方法收集2010年至2012年经手术病理证实的无肿块乳头溢液患者有CR钼靶乳腺导管造影28例进行回顾性分析,术前按照美国放射学会第四版制定的BI-RADS进行分类。结果 28例患者,BIRADS分类2类10例,其中炎症并导管扩张7例,导管内乳头状瘤3例;3类8例,导管内乳头状瘤5例,导管内乳头状瘤病2例,乳腺癌1例;4类9例,炎症并导管扩张1例,乳腺癌8例;5类1例,乳腺癌1例。术前误诊2例,诊断符合率为92.9%。结论乳腺导管造影结合BI-RADS分级系统能够提高无肿块乳头溢液诊断敏感性和特异性。
Objective To investigate the clinical value of mammography-selective mammography combined with video report and data system (BI-RADS) in the diagnosis of breast disease without mass nipple discharge. Methods A total of 28 patients with luminal papillary discharge without lumpectomy confirmed by surgery and pathology from 2010 to 2012 were retrospectively analyzed. BI-RADS classification was established preoperatively according to the fourth edition of the American College of Radiology. Results Of the 28 patients, 10 were classified into 2 categories according to BIRADS classification, including 7 cases of inflammation and catheter expansion, 3 cases of intraductal papilloma, 8 cases of 3 types, 5 cases of intraductal papilloma, 2 cases of intraductal papilloma, 1 case of breast cancer, 4 cases of 9 cases, 1 case of inflammation and catheter dilatation, 8 cases of breast cancer, 1 case of 5 cases and 1 case of breast cancer. Preoperative misdiagnosis in 2 cases, the diagnostic coincidence rate was 92.9%. Conclusion Breast ductography combined with BI-RADS grading system can improve the sensitivity and specificity of the diagnosis without lump nipple discharge.
