目的 检测65例妊娠晚期子痫前期孕妇可溶性血管内皮生长因子受体-1(sFlt-1)、精氨酸加压素水平(AVP)和凝血功能,为患者的临床诊断提供指导.方法 选择2017年8月-2019年3月天津泰达医院收治的65例妊娠晚期子痫前期患者为子痫前期组,根据病情严重程度分为轻度子痫前期组(29例)和重度子痫前期组(36例).另选择同期在天津市泰达医院就诊的35例正常妊娠晚期孕妇为对照组.比较各组血清sFlt-1、AVP水平及凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原(FIB)水平等凝血功能指标.结果 4组孕妇血清sFlt-1、AVP水平,PT、APTT、TT、FIB水平比较差异有统计学意义(P＜0.05).子痫前期组、重度子痫前期组、轻度子痫前期组孕妇血清sFlt-1、AVP水平均明显高于对照组(均P＜0.05).重度子痫前期组孕妇血清sFlt-1、AVP水平均明显高于轻度子痫前期组(均P＜0.05).子痫前期组、重度子痫前期组、轻度子痫前期组孕妇PT、APTT、TT均明显低于对照组(均P＜0.05),子痫前期组、重度子痫前期组、轻度子痫前期组孕妇FIB水平明显均高于对照组(均P＜0.05).重度子痫前期组孕妇PT、APTT、TT均明显低于轻度子痫前期组(均P＜0.05),FIB水平明显高于轻度子痫前期组(P＜0.05).结论 血清sFlt-1、AVP水平及机体凝血功能与妊娠晚期子痫前期的发生、发展有关,妊娠晚期子痫前期孕妇处于高凝血状态及血栓前状态,且随着病情加重而越发明显.加强对上述指标的动态检测对于妊娠晚期子痫前期的预后评估及临床防治具有重要意义.
