目的:分析影响颅脑损伤昏迷患者预后的因素。方法:对我院神经外科2005-2007年收治的108例颅脑损伤所致昏迷患者的临床资料进行回顾性分析。结果:对71例患者行头颅CT检查,结果表明,昏迷时间小于半小时的浅昏迷者占11.3%(8/71),其预后良好;深昏迷者占88.7%(63/71),其中,脑内血肿、硬膜下血肿和蛛网膜下腔出血明显增加了颅脑损伤的死亡率。结论:颅脑损伤所致昏迷患者在半小时内进行抢救,其一般预后较好,而格拉斯哥昏迷评分(GCS)<5分者,病死率高达96%(;GCS)<8分的双侧半球损伤和脑室损伤患者,其一般预后较差;伴有硬膜下血肿、脑内血肿和蛛网膜下腔出血的颅脑昏迷者,预后也较差。
OBJECTIVE: To analyze the factors that affect the prognosis of patients with brain injury and coma. Methods: A retrospective analysis was performed on the clinical data of 108 patients with coma caused by traumatic brain injury admitted to our department from 2005 to 2007 in Department of Neurosurgery. Results: A craniofacial CT examination was performed on 71 patients. The results showed that 11.3% (8/71) of coma were in coma time less than half an hour, with a good prognosis; 88.7% (63/71) in deep coma, Intracerebral hematoma, subdural hematoma and subarachnoid hemorrhage significantly increased the mortality rate of head injury. CONCLUSIONS: Patients with coma caused by craniocerebral injury are usually rescued within half an hour and generally have a better prognosis. Patients with Glasgow coma score <5, patients with a mortality rate of up to 96% (GCS) <8, Injuries and ventricular injury in patients with its general prognosis is poor; with subdural hematoma, intracerebral hematoma and subarachnoid hemorrhage of cerebral coma, the prognosis is poor.
