频谱心电图(FCG)是一种诊断冠心病的新方法。但是,FCG在诊断早期冠心病中的可靠性还缺乏实验资料。为了评价FCG在诊断早期冠心病中的敏感性和特异性,并为临床提供实验依据,本文观察了阻断家兔冠状动脉左旋支和再灌流以及结扎犬的冠状动脉左旋支前后,缺血区心室肌细胞动作电位(AP)、心电图(ECG)和FCG的变化。缺血区心室肌AP记录方法采用Woodburg的浮置微电极技术。ECG和FCG记录用HBD—Ⅱ型心电多域信息自动诊断仪,以常规ECG的V_5(X通道)和Ⅱ(Y通道)连续同步采样125s,由仪器自动绘出图形。结果可见,在冠状动脉阻断10min后,动作电位幅度(APA)降低,O相上升速率(Vmax)减慢,动作电位时程APD_(30)和APD_(50)明显缩短。ECG表现为ST段和T波抬高。FCG的主要改变为自功率谱(Gxx、Gyy)的第一峰幅度明显增加,自相关函数(Vxx、Vyy)和互相关函数(Vxy)中的R波幅增大,R波的衰减不明显,并且R波间的小波动消失。在再灌流后10min、AP、ECG和FCG恢复到接近正常。结扎犬的冠状动脉左旋支后,FCG的改变与阻断家兔冠状动脉左旋支结果相似。这些结果提示,ECG对冠状动脉梗塞早期的心肌电活动变化是敏感的.
Spectrum electrocardiogram (FCG) is a new method of diagnosing coronary heart disease. However, the reliability of FCG in the diagnosis of early coronary heart disease is still lack of experimental data. In order to evaluate the sensitivity and specificity of FCG in the diagnosis of early coronary heart disease and to provide experimental evidence for clinical study, we observed the effects of blocking the left circumflex coronary artery in left circumflex coronary artery, Changes of action potential (AP), electrocardiogram (ECG) and FCG of ventricular myocytes. The ischemic ventricular AP recording method uses Woodburg’s floating microelectrode technique. ECG and FCG recording with HBD-Ⅱ type ECG multi-domain automatic diagnostic apparatus, with conventional ECG V_5 (X channel) and Ⅱ (Y channel) continuous synchronous sampling 125s, automatic drawing by the instrument graphics. The results showed that after the coronary artery was blocked for 10 minutes, the action potential amplitude (APA) decreased and the O phase rise rate (Vmax) slowed down. The action potential duration APD_ (30) and APD_ (50) significantly shortened. ECG showed ST segment and T wave elevation. The main change of FCG is that the amplitude of the first peak of self-power spectrum (Gxx, Gyy) increases obviously, the amplitude of R in autocorrelation function (Vxx, Vyy) and cross-correlation function (Vxy) increases, the attenuation of R wave is not obvious, And small fluctuations between R waves disappear. At 10 min after reperfusion, AP, ECG and FCG returned to near normal. Ligation of canine left circumflex coronary artery, FCG changes and blocking rabbit left circumflex coronary artery similar results. These results suggest that ECG is sensitive to changes in cardiac electrical activity during the early stages of coronary infarction.
