目的观察糖尿病大鼠股骨头微血管分布及形态上的变化。探讨糖尿病大鼠股骨头组织血管内皮生长因子(VEGF)mRNA在不同时间表达的差异以及醛糖还原酶(AR)活性变化对股骨头组织结构演变、微血管病变的影响。方法建立速发型链脲佐菌素(STZ)糖尿病大鼠模型,随机分为正常5周组(CON1)、10周组(CON2)及15周组(CON3),糖尿病5周组(DM1)、10周组(DM2)及15周组(DM3),每组10只。光镜下观察各组股骨头结构以及凝血因子Ⅷ免疫组织化学表达情况;墨汁灌注行微血管密度测定;观察VEGF mRNA原位杂交表达强度并分析;生化检测各组股骨头组织匀浆的AR活性。结果糖尿病大鼠股骨头随病程发展骨小梁变少变薄,骨髓腔增大;凝血因子Ⅷ因子表达上升,微血管密度增大;VEGF mRNA在骨髓腔血管内皮细胞表达上升,AR活性增大(P<0.05)。结论糖尿病大鼠股骨头出现骨质疏松变化倾向,与VEGF mRNA促进微血管增生、血管通道性增大及AR活性增强密切相关。
Objective To observe the distribution and morphological changes of femoral head capillaries in diabetic rats. To investigate the difference of expression of vascular endothelial growth factor (VEGF) mRNA in femoral head tissue and the effect of aldose reductase (AR) activity on the structure and microangiopathy of femoral head in diabetic rats. Methods A rat model of immediate-onset streptozotocin (STZ) was established and randomly divided into 5 groups: CON1, CON2, CON3, DM1, 10 weeks group (DM2) and 15 weeks group (DM3), 10 rats in each group. The structure of femoral head and the immunohistochemical expression of factor Ⅷ were observed under light microscope. The microvessel density was measured by ink perfusion. The expression of VEGF mRNA in situ hybridization was observed and analyzed. The AR activity of femoral head homogenate was detected by biochemical method. Results The trabecular bone became thinner and the bone marrow cavity increased with the development of the course of the disease. The expression of factor Ⅷ increased and the density of microvessel increased. The expression of VEGF mRNA increased in the vascular endothelial cells of the medullary cavity and the activity of AR increased P <0.05). Conclusion There is a tendency of osteoporosis in the femoral head of diabetic rats, which is closely related to the promotion of microvessel hyperplasia, vascular access and AR activity enhancement.
