目的:检测人B细胞激活因子(B lymphocyte stimulator,BlyS)在原发性干燥综合征(primary Sjgrensyndrome,pSS)患者肾组织中的表达,并比较在高丙种球蛋白(IgG)与丙种球蛋白正常两组患者中BlyS表达的差异。方法:选取上海市瑞金医院肾脏科住院诊断为pSS,且合并肾脏损害的34例患者的肾活检标本,按血清IgG水平,分为高IgG组(IgG≥1660mg/dl)和正常IgG组(IgG<1660mg/dl),应用免疫荧光法观察BlyS在肾组织中的表达,以5例正常肾脏组织作为对照,采用one way ANOVA分析法比较在高IgG与正常IgG两组患者中BlyS表达的差异。结果:免疫荧光结果显示:(1)BlyS在正常肾脏组织中,无或仅少量BlyS表达;而在pSS患者肾脏组织中可见浸润的炎性细胞特异性荧光亮染,BlyS主要表达细胞膜上。在肾脏小管间质区和肾小球中浸润的炎细胞中圴有BlyS的表达。(2)高IgG组与正常IgG两组患者肾脏组织BlyS表达的差异以荧光强度反映BlyS表达差异,发现正常IgG组患者肾组织BlyS表达高于高IgG组(P<0.05)。结论:pSS患者肾脏组织BlyS表达显著高于正常对照,其不仅表达在肾小管间质浸润的炎性细胞表面,而且在肾小球炎性细胞中也有表达。
OBJECTIVE: To detect the expression of B lymphocyte stimulator (BlyS) in the kidney of patients with primary Sjögrens syndrome (pSS) and to compare the expression of B lymphocyte stimulator (B lymphocyte stimulator, Difference of BlyS expression in two groups of normal protein. Methods: The renal biopsy specimens of 34 patients diagnosed as pSS with renal damage in hospital of nephrology department of Ruijin Hospital of Shanghai were divided into high IgG group (IgG≥1660mg / dl) and normal IgG group (IgG <1660mg / dl). The expression of BlyS in renal tissues was observed by immunofluorescence. Five normal renal tissues were used as control. The difference of BlyS expression between two groups of high IgG and normal IgG was compared by one way ANOVA. Results: The results of immunofluorescence showed that: (1) BlyS was expressed in normal kidney tissue with little or only BlyS; while infiltrating inflammatory cell-specific fluorescence was observed in renal tissues of pSS patients. BlyS was mainly expressed on the cell membrane. BlyS expression is present in inflammatory cells infiltrating the tubulointerstitium and glomerulus. (2) The difference of BlyS expression in renal tissue between high IgG group and normal IgG groupByS fluorescence intensity reflected the difference of BlyS expression, and found that the expression of BlyS in renal tissue of normal IgG group was higher than that of high IgG group (P <0.05). Conclusion: The expression of BlyS in renal tissues of patients with pSS is significantly higher than that of normal controls. The expression of BlyS is not only expressed on the surface of inflammatory cells infiltrating into the tubulointerstitium, but also in glomerular inflammatory cells.
