星状神经节阻滞常用于某些疾病的诊断和治疗,包括治疗手外伤。本文报告一例男性51岁患者,一个月前右手腕及前臂压伤,进行星状神经节阻滞治疗,先做过两次阻滞未发生意外,第三次阻滞时病人并无恐惧情绪。本次阻滞取气管旁进路,插入25G 穿刺针,回抽无血后准备注入10毫升0.25%布比卡因(Bupivacaine)。当注入1毫升时,患者开始呻吟,移动手臂,用手指面部。发现其面色苍白,双侧瞳孔扩大,不能讲话。1分钟后,患者恢复讲话时说失明了。当时意识清楚,无惊厥,呼吸心率正常。约5分钟后,患者视力恢复。事后患者说,由于舌和下颁无力而不能讲话,提到其视力丧失,起初四周黑暗,然后看到无数微小的闪光亮点,
