目的研究改良超滤法浓缩血液的效果及其对体内部分细胞因子(TNF、IL-8、ET)的影响。方法40例小儿先心病患者,随机分为两组(实验组和对照组),每组20例。体外循环中维持血球压积20%,胶体渗透压1.33kPa左右。实验组病人在停机后,拔管前行改良法超滤。结果超滤后,病人的血细胞及蛋白等大分子物质的浓度明显提高,而IL-8、ET和尿素氮、肌酐这些中小分子物质的浓度经白蛋白校正后明显降低,TNF浓度无明显变化。结论应用改良超滤法可以迅速浓缩血液中的细胞和蛋白,在滤出水份同时,还能有效的去除多种有害中分子物质。
Objective To study the effect of modified ultrafiltration on blood concentration and its effect on some cytokines (TNF, IL-8 and ET) in vivo. Methods 40 cases of children with congenital heart disease were randomly divided into two groups (experimental group and control group), 20 cases in each group. Cardiopulmonary bypass to maintain hematocrit 20%, colloid osmotic pressure of about 1.33kPa. Experimental group of patients in the shutdown, extubation before improvement ultrafiltration. Results After ultrafiltration, the concentration of macromolecules such as blood cells and protein in patients increased significantly. However, the concentrations of small molecules such as IL-8, ET, urea nitrogen and creatinine were significantly reduced after albumin correction, but the concentration of TNF did not change significantly. Conclusion The modified ultrafiltration method can quickly concentrate the blood cells and proteins, in the filtered water, but also effectively remove a variety of harmful intermediate substances.
