目的:探讨胶体32P与p53基因治疗肺癌胸腔积液的近期疗效和毒副作用。方法:65例确诊为肺癌并发单侧癌性胸腔积液的患者,随机分为3组。A组用重组人p53腺病毒注射液联合胶体32 P-磷酸铬(胶体32 P)治疗,B组单用胶体32P,C组单用重组人p53腺病毒注射液治疗。各入组者均每月治疗1次,治疗2次后评价疗效。结果:A组总有效率为81.8%,B组总有效率为52.4%,C组总有效率为40.9%,B组与C组之间比较,差异无统计学意义,χ2=0.57,P>0.05;B、C组分别与A组比较,差异均有统计学意义,χ2值分别为4.24和7.77,P值均<0.05;3组患者恶心、呕吐和粒细胞减少差异无统计学意义,A和C组均有5例发热,与B组无发热比较,差异有统计学意义,P值均为0.048。结论:单独使用胶体32P或p53基因治疗肺癌胸腔积液疗效有限;胶体32P与p53基因联合治疗可以有效地控制肺癌胸腔积液。
Objective: To investigate the short-term curative effect and side effects of colloidal 32P and p53 gene in treatment of pleural effusion of lung cancer. Methods: Sixty-five patients diagnosed as lung cancer with unilateral malignant pleural effusion were randomly divided into three groups. Group A was treated with recombinant human p53 adenovirus injection in combination with colloidal chromium 32-phosphate (colloid 32 P), group B with colloid 32P alone and group C with recombinant human p53 adenovirus alone. Each group were treated once a month, 2 times after treatment to evaluate the efficacy. Results: The total effective rate in group A was 81.8%, the total effective rate in group B was 52.4%, the total effective rate in group C was 40.9%, there was no significant difference between group B and group C (χ2 = 0.57, P> 0.05 respectively. There was significant difference between group B and group C and group A respectively (χ2 = 4.24 and 7.77, P <0.05 respectively). There was no significant difference in nausea, vomiting and neutropenia among group A and group A And C group had 5 cases of fever, no fever compared with the B group, the difference was statistically significant, P values were 0.048. Conclusions: Colloid 32P or p53 gene therapy alone has limited efficacy in the treatment of pleural effusion in lung cancer. Colloid 32P and p53 gene therapy can effectively control pleural effusion in lung cancer.
