目的探讨功能性消化不良(FD)患儿胃黏膜肥大细胞(MC)的密度及其活化状态与胃排空的关系。方法按罗马Ⅱ标准选择FD患儿40例,每例患儿均取胃窦黏膜1块,应用免疫组织化学法检测其MC数及其脱颗粒数,然后行固体胃排空检测,固体胃排空正常患儿为胃排空正常组,其他患儿为胃排空延迟组;统计所有标本MC的密度及脱颗粒程度,比较二组患儿的MC密度及活化状态的差异,比较MC密度正常患儿与异常患儿的胃排空差异,评估MC的脱颗粒指数与胃排空间的关系,最后进行胃排空与MC密度及脱颗粒指数间的相关性分析。结果 1.FD患儿的胃黏膜MC脱颗粒指数为(65.7±23.9)%,其中>50%者占82.5%;2.与1 h胃排空正常组相比,1 h胃排空延迟组MC密度显著升高(P=0.010);3.与MC密度正常组相比,MC密度异常组1 h胃排空明显延迟(P=0.001);4.MC脱颗粒指数与胃排空无明显关系;5.MC密度与1 h胃排空率呈负相关(P=0.030)。结论 FD患儿的胃排空延迟与MC密度升高有关,MC可能通过影响胃排空介导儿童FD的发生和发展。
Objective To investigate the relationship between gastric mucosal mast cells (MC) density and activation state and gastric emptying in children with functional dyspepsia (FD). Methods According to the criteria of Rome Ⅱ, 40 children with FD were selected. One child with gastric mucosa was taken from each child. The number of MC and its number of degranulation were detected by immunohistochemical method. Then the gastric emptying, Normal children with empty stomach were normal in gastric emptying group and others were delayed gastric emptying group. The density and degree of degranulation of MC in all specimens were statistically analyzed. The differences of MC density and activation state between two groups were compared. Differences in gastric emptying between children and children with abnormalities were evaluated to evaluate the relationship between MC degranulation index and gastric emptying space. Finally, the correlation between gastric emptying and MC density and degranulation index was analyzed. Results 1. The MC degranulation index of gastric mucosa in children with FD was (65.7 ± 23.9)%, of which> 50% was 82.5%. 2. Compared with the normal gastric emptying group, the delayed gastric emptying group MC density was significantly increased (P = 0.010); 3. Compared with the normal MC group, the gastric emptying was significantly delayed in the group with abnormal MC density at 1 hour (P = 0.001); 4.MC degranulation index had no significant difference with gastric emptying Relationship; 5.MC density and 1 h gastric emptying rate was negatively correlated (P = 0.030). Conclusion Delayed gastric emptying in FD children is related to the increase of MC density. MC may influence the occurrence and development of FD in children by affecting gastric emptying.
