目的:分析支气管镜下氩等离子体凝固术(APC)联合冷冻治疗中央型肺癌伴大气道阻塞疗效。方法:选取2014年12月至2016年11月我院接受治疗气道梗阻的肺癌患者120例,根据患者意愿分为对照组和观察组,每组患者各60例。对照组给予常规化疗,观察组在常规化疗基础上联合支气管镜下APC联合冷冻介入治疗。观察和评价两组患者治疗后临床效果、生存期、Karnofsky评分及安全性。结果:观察组60例患者治疗后完全有效22(36.7%)例、部分有效32(53.3%)例、总有效率高达90.0%,显著优于对照组(P<0.05)。观察组患者生存期和Karnofsky评分均高于对照组,差异有统计学意义(P<0.05)。APC联合冷冻介入治疗安全性较好。结论:支气管镜下氩等离子体凝固术联合冷冻治疗中央型肺癌伴大气道阻塞疗效较单纯化疗突出,安全性好,可延长生存期,值得临床推广应用。
Objective: To analyze the curative effect of bronchoscopic argon plasma coagulation (APC) combined with cryotherapy on central lung cancer with obstruction of the airway. Methods: From December 2014 to November 2016, 120 patients with lung cancer undergoing airway obstruction in our hospital were selected and divided into control group and observation group according to patients’ wishes, 60 patients in each group. The control group was given conventional chemotherapy. The observation group was treated with bronchoscopic APC combined with cryogenic intervention based on routine chemotherapy. The clinical effects, survival, Karnofsky score and safety of the two groups were observed and evaluated. Results: In the observation group, 60 patients were completely effective after treatment, 22 (36.7%) cases and 32 (53.3%) cases were partially effective. The total effective rate was as high as 90.0%, which was significantly better than that of the control group (P <0.05). Survival and Karnofsky scores in the observation group were higher than those in the control group, the difference was statistically significant (P <0.05). APC combined with frozen intervention is safe. Conclusion: Bronchoscopic argon plasma coagulation combined with cryotherapy for central lung cancer with airway obstruction is more effective than simple chemotherapy, which is safe and prolongs survival. It is worthy of clinical application.
