目的研究经尿道灌注肝素对导管伴随性尿路感染(UTIc)的影响。方法将住院的176例尿路带管病人随机分成4组,经尿管灌注生理盐水(C组)、庆大霉素(G组)、肝素钠(H组)及肝素与庆大霉素交替(M)组,于置管后不同时间留尿或取导管培养,对导管内表面扫描电镜观察。结果留管1~3周C组和G组尿培养阳性率明显高于H组和M组,留管>1周后C组和G组导管表面菌落数明显高于H组和M组。扫描电镜观察显示H组和M组比C组和G组导管表面附着层薄、纤维蛋白网稀、细菌密度低。结论经尿管灌注肝素能够预防或延迟导管伴随性尿路感染的发生。
Objective To investigate the effect of transurethral heparin infusion on catheter-associated urinary tract infection (UTIc). Methods A total of 176 patients with urinary tract obstruction in hospital were randomly divided into four groups. The rats in group C received gentamicin (G), heparin (H group), heparin and gentamycin (M) group. After catheterization, urine or catheter culture was taken at different times after catheterization, and the inner surface of the catheter was observed by scanning electron microscopy. Results The positive rate of urinary culture in C group and G group after 1 to 3 weeks of storage was significantly higher than that of H group and M group. After 1 week of storage, the number of colonies on C group and G group was significantly higher than that of H group and M group. Scanning electron microscopy showed that H and M groups were thinner than those of C and G groups, and the fibrin network was thin and their bacterial density was low. Conclusion Transcatheter heparin infusion can prevent or delay the occurrence of catheter-associated urinary tract infection.
