目的对序贯机械通气治疗慢性阻塞性肺疾病(COPD)合并严重Ⅱ型呼吸衰竭的疗效进行分析。方法所有患者随机分为二组,序贯机械通气治疗组11例,对照组10例。将序贯治疗组和对照组各个时间点的动脉血pH、PaO2及PaCO2进行比较,并观察两组治疗前后的pH、PaO2及PaCO2变化。结果序贯治疗组与对照组在各个时间点的动脉血pH、PaO2及PaCO2比较无明显差异(P>0.05);两组的动脉血pH、PaO2及PaCO2在治疗后24小时有明显改善(P<0.05),治疗后72小时有显著改善(P<0.01);两组的症状和体征均获得改善。结论序贯机械通气是一种有效的治疗COPD合并严重Ⅱ型呼吸衰竭的手段。
Objective To analyze the curative effect of sequential mechanical ventilation on chronic obstructive pulmonary disease (COPD) complicated with severe type Ⅱ respiratory failure. Methods All patients were randomly divided into two groups: sequential mechanical ventilation in 11 cases and control group in 10 cases. The arterial blood pH, PaO2 and PaCO2 were compared between sequential treatment group and control group at different time points. The changes of pH, PaO2 and PaCO2 before and after treatment were observed. Results There was no significant difference in arterial blood pH, PaO2 and PaCO2 between the sequential therapy group and the control group at each time point (P> 0.05). The arterial blood pH, PaO2 and PaCO2 in both groups were significantly improved at 24 hours after treatment <0.05), and there was a significant improvement at 72 hours after treatment (P <0.01). The symptoms and signs of both groups were improved. Conclusions Sequential mechanical ventilation is an effective treatment of COPD with severe type II respiratory failure.
