目的了解2010-2014年上海市382株志贺菌对抗菌药物的敏感性及其毒力基因的携带情况。方法采用纸片扩散法(kirby-bauer,K-B)检测139株福氏志贺菌和243株宋内志贺菌对13种抗菌药物的敏感性;同时对志贺菌的五种毒力基因进行聚合酶链式反应(polymerase chain reaction,PCR)扩增检测。结果菌株对萘啶酸、链霉素、氨苄西林、四环素和复方新诺明的耐药率分别达到了97.4%、96.1%、84.8%、77.7%和77.2%,多重耐药情况严重。福氏志贺菌对氨苄西林、头孢噻肟、头孢吡肟、氯霉素、阿莫西林/克拉维酸、四环素、环丙沙星、诺氟沙星及左氧氟沙星的耐药率均高于宋内志贺菌,差异均具有统计学意义(均有P<0.05)。382株志贺菌全部携带ipa H基因,其中63.3%的福氏志贺菌同时携带ipa H、ial、sen、set1A和set1B这5种毒力基因,50.6%的宋内志贺菌同时携带ipa H、ial和sen这3种毒力基因。结论上海地区志贺菌的耐药情况严重,且福氏和宋内志贺菌对部分抗菌药物的抗药性差别较大;福氏志贺菌毒力基因携带率高,毒力强,应注意防范。
Objective To investigate the susceptibility of antimicrobial agents and virulence genes of 382 Shigella strains from 2010 to 2014 in Shanghai. Methods The sensitivity of 13 strains of Shigella flexneri and 243 strains of Shigella sonnei to 13 antimicrobial agents was detected by Kirby-Bauer method (KB). Five virulence genes of Shigella Polymerase chain reaction (PCR) amplification detection. Results The resistance rate of the strains to nalidixic acid, streptomycin, ampicillin, tetracycline and cotrimoxazole reached 97.4%, 96.1%, 84.8%, 77.7% and 77.2% respectively, and the multi-drug resistance was serious. The resistance rates of Shigella flexneri to ampicillin, cefotaxime, cefepime, chloramphenicol, amoxicillin / clavulanic acid, tetracycline, ciprofloxacin, norfloxacin and levofloxacin were all higher than Song Shigella, the differences were statistically significant (both P <0.05). 382 Shigella strains all carried ipa H gene, of which 63.3% of Shigella flexneri simultaneously carried 5 virulence genes of ipa H, ial, sen, set1A and set1B, while 50.6% Shigella sonnei simultaneously carried ipa H, ial and sen these three virulence genes. Conclusion The resistance of Shigella in Shanghai is serious, and Shigella flexneri and Shigella flexneri have different resistance to some antimicrobial agents. Shigella flexneri virulence gene is highly virulent and virulent, we should pay attention Prevent.
