目的描述2010—2014年在北京口岸进行出境前健康体检的男性出国劳务人员艾滋病毒(HIV)抗体阳性率,并分析影响HIV感染的因素。方法资料来自北京国际旅行卫生保健中心信息系统。按照相关的纳入和排除标准,选取北京口岸男性出国劳务人员为研究对象,获取其人口学资料和HIV抗体检测数据。对计数资料采用均值±标准差进行描述;对计量资料采用率、构成比等对结果进行描述,采用logistic回归对影响因素进行分析。结果纳入分析的研究对象共26 843人,检测出HIV抗体阳性17人,阳性率为63.3/10万(95%CI:33.2~93.4/10万)。18~29岁组、30~39岁组和40~65岁组阳性率分别为75.0/10万、60.2/10万和50.8/10万;来自东部地区、中部地区和西部地区组阳性率分别为41.6/10万、50.5/10万和176.8/10万;2010—2014年阳性率分别为22.0/10万、82.2/10万、159.2/10万、41.8/10万和54.3/10万。多因素Logistic回归表明,来自西部地区的男性出国劳务人员HIV感染风险是来自东部地区的4.200倍;不同年龄组男性出国劳务人员HIV感染风险差异不具有统计学意义(P>0.05)。结论北京口岸男性出国劳务人员HIV抗体阳性率较高,应关注该群体健康状况,尤其是来自西部地区的男性出国劳务人员的HIV感染状况。
Objective To describe the prevalence of HIV antibodies among male migrant workers who carried out physical examination before departure at the Beijing port from 2010 to 2014 and analyze the factors influencing HIV infection. Method information from Beijing International Travel Health Care Center Information System. In accordance with the relevant inclusion and exclusion criteria, male laborers from Beijing Port were selected as research subjects to obtain their demographic data and HIV antibody testing data. The count data were described as mean ± standard deviation. The results were described with the adoption rate of measurement data and composition ratio. The influencing factors were analyzed by logistic regression. Results A total of 26 843 subjects were enrolled in the analysis. Totally 17 HIV-positive individuals were detected, with a positive rate of 63.3 / 100 000 (95% CI: 33.2-93.4 / 100 000). The positive rates were 75.0 / 100000, 60.2 / 100000 and 50.8 / 100000 in the groups of 18-29 years, 30-39 years and 40-65 years old respectively. The positive rates in the eastern, central and western regions were 41.6 / 100000, 50.5 / 100000 and 176.8 / 100000; the positive rates in 2010-2014 were 22.0 / 100000, 82.2 / 100000, 159.2 / 100000, 41.8 / 100000 and 54.3 / 100000 respectively. Multivariate logistic regression showed that the risk of HIV infection among men who came from abroad in the western region was 4.200 times that of the eastern region. There was no significant difference in the risk of HIV infection among men of different ages abroad (P> 0.05). Conclusions The positive rate of HIV antibody among male laborers who go abroad at Beijing Port is high. We should pay attention to the health status of the population, especially the HIV infection status of male migrant workers from the western region.
