目的评价高频振荡通气在治疗新生儿呼吸窘迫综合征的疗效。方法将NRDS患儿106例随机分为高频振荡通气组(HFOV组)和常频机械通气组(CMV组),比较两组治疗效果、通气后6 h和24 h患儿血气分析结果以及住院时间等指标。结果 HFOV组治愈率显著高于CMV组(P<0.05);且HFOV组患儿在通气后6 h和12 h,PaO2高于CMV组,OI、PaCO2低于CMV组,差异均有统计学意义(P<0.01);HFOV组患儿住院时间显著少于CMV组(P<0.01)。结论高频振荡通气较常频机械通气更能有效改善呼吸窘迫综合征患儿肺功能情况,提高治愈率,减少患儿住院时间,值得推广应用。
Objective To evaluate the efficacy of high frequency oscillatory ventilation in the treatment of neonatal respiratory distress syndrome. Methods 106 children with NRDS were randomly divided into two groups: high-frequency oscillatory ventilation group (HFOV group) and normal-frequency mechanical ventilation group (CMV group). The therapeutic effect was compared between the two groups. Results of blood gas analysis at 6 h and 24 h after ventilation and hospitalization Time and other indicators. Results The cure rate of HFOV group was significantly higher than that of CMV group (P <0.05). In HFOV group, PaO2 was higher at 6 h and 12 h after ventilation than at CMV group, while OI and PaCO2 were lower than CMV group (P <0.01). The hospitalization time in HFOV group was significantly less than that in CMV group (P <0.01). Conclusions High-frequency oscillatory ventilation is more effective than conventional mechanical ventilation in improving lung function, improving the cure rate and reducing hospitalization time in children with respiratory distress syndrome, which is worth popularizing and applying.
