目的探讨激光辅助孵化(laser-assisted hatching,LAH)对种植失败患者冻融胚胎移植(frozen-thawed embryo transfer,FET)周期妊娠结局的影响。方法 2012年5月至2013年12月在本中心接受辅助生殖治疗的97例胚胎种植失败患者,选取其124个FET周期并随机分为两组:孵化组(61个周期),胚胎移植前行LAH;未孵化组(63个周期),胚胎移植前未行辅助孵化。比较两组的临床妊娠率和胚胎种植率以及流产率、多胎率。结果孵化组的临床妊娠率(47.5%)、胚胎种植率(29.2%)均高于未孵化组(30.2%和19.0%),差异有统计学意义(P<0.05)。流产率、多胎率等结局指标在两组间的差异无统计学意义(P>0.05)。结论 LAH可明显提高种植失败患者FET周期临床妊娠率和胚胎种植率,改善临床结局。
Objective To investigate the effect of laser-assisted hatching (LAH) on the pregnancy outcome of frozen-thawed embryo transfer (FET) in patients with failed implantation. Methods A total of 97 embryo implantation failure patients receiving assisted reproductive therapy in our center from May 2012 to December 2013 were selected and randomly divided into two groups: hatching group (61 cycles), embryo transfer LAH; unhatched group (63 cycles), no hatching assisted before embryo transfer. The clinical pregnancy rate, embryo implantation rate, abortion rate and multiple birth rate were compared between the two groups. Results The clinical pregnancy rate (47.5%) and embryo implantation rate (29.2%) in hatching group were significantly higher than those in un hatching group (30.2% and 19.0%, respectively). The difference was statistically significant (P <0.05). Miscarriage rate and multiple pregnancy rate had no significant difference between the two groups (P> 0.05). Conclusions LAH can significantly improve the clinical pregnancy rate and embryo implantation rate in FET cycles and improve the clinical outcomes in implant failure patients.
