目的通过流式细胞仪对卵巢癌性腹水脱落细胞进行DNA倍体分析,并同血清CA125检测方法相比较,以评估二种方法及其联合检测在卵巢癌诊断中的价值。方法取2006年3月至2008年1月吉林大学第二医院妇产科及长春市结核病院卵巢癌患者腹水30例、结核性腹水15例。使用流式细胞仪对腹水脱落细胞进行DNA倍体分析,并同时检测血清CA125。结果卵巢癌性腹水组增殖指数(proliferation index,PI)、DNA指数(DNA in-dex,DI)均高于结核性腹水组,差异有统计学意义(P(0.05)。DNA倍体分析诊断卵巢癌的灵敏度为73.33%,特异度为93.33%;以血清CA125诊断卵巢癌的灵敏度为76.67%,特异度为26.67%。两种方法灵敏度差异无统计学意义(P(0.05),特异度差异有统计学意义(P(0.01)。结论使用流式细胞仪DNA倍体分析检测卵巢癌性腹水简单、快速且具有较高的灵敏度和特异度。如果联合血清CA125检测,对卵巢癌性腹水有更好的诊断价值。
Objective To evaluate the value of two methods and their combined detection in the diagnosis of ovarian cancer by DNA ploid analysis of ovarian cancer ascites exfoliated cells by flow cytometry and compared with serum CA125 detection method. Methods From March 2006 to January 2008, 30 cases of ascites and 15 cases of tuberculous ascites were collected from Department of Obstetrics and Gynecology, Second Hospital of Jilin University and Changchun Tuberculosis Hospital. Flow cytometry was used for DNA ploid analysis of ascites exfoliated cells and simultaneous detection of serum CA125. Results The proliferation index (PI) and DNA in-dex (DI) in ovarian ascites group were significantly higher than those in tuberculous ascites group (P <0.05) The sensitivity and specificity of the two methods were 73.33% and 93.33%, respectively. The sensitivity and specificity of serum CA125 in diagnosing ovarian cancer were 76.67% and 26.67%, respectively. There was no significant difference between the two methods in the sensitivity (P <0.05) (P0.01) .Conclusion The detection of ovarian cancer ascites by flow cytometry DNA ploid analysis is simple, rapid and has high sensitivity and specificity.If combined with serum CA125 detection, there is more for ovarian cancer ascites Good diagnostic value.
