目的探讨无创呼吸机治疗急性左心衰的临床效果。方法选取2014年6月—2016年6月本院收治的68例急性左心衰患者,采用随机数字表法分为观察组和对照组各34例,观察组给予Bi PAP无创双水平气道正压呼吸机经口鼻面罩辅助通气治疗;对照组给予面罩吸氧。观察两组患者的生命体征、治疗有效率及体力稳定率情况。计量资料组间比较采用两独立样本t检验,组内比较采用配对t检验;计数资料比较采用χ2检验,P<0.05为差异有统计学意义。结果治疗后两组患者的心率(heart rate,HR)、呼吸频率(respiratory rate,RR)、平均动脉压(mean arterial pressure,MAP)、血氧饱和度(oxyhemoglobin saturation,Sa O2)、动脉氧分压(arterial oxygen pressure,Pa O2)、动脉二氧化碳分压(arterial carbon dioxide pressure,Pa CO2)、收缩压(systolic blood pressure,SBP)、舒张压(diastolic blood pressure,DBP)比较,差异均有统计学意义(均P<0.05)。治疗前后观察组患者的各指标比较,差异均有统计学意义(均P<0.05);治疗后对照组患者的HR、RR、MAP、Sa O2、Pa O2、SBP比较,差异均有统计学意义(均P<0.05)。观察组患者的总有效率为88.24%,显著高于对照组的67.65%,差异有统计学意义(P<0.05)。观察组稳定率为94.12%(32/34),对照组为70.59%(24/34),比较差异有统计学意义(P<0.05)。结论无创呼吸机可有效改善急性左心衰患者的生命体征和体力状况,效果显著,值得临床推广及应用。
Objective To investigate the clinical effect of noninvasive ventilator on acute left heart failure. Methods Sixty-eight patients with acute left heart failure admitted to our hospital from June 2014 to June 2016 were randomly divided into observation group (34 cases) and control group (34 cases). The observation group was given Bi PAP Pressure ventilator oral nasal mask assisted ventilation; control group given mask oxygen. Two groups of patients were observed vital signs, treatment efficiency and physical stability rate. Measurement data were compared between groups using two independent samples t test, the group was compared using paired t test; count data were compared using the χ2 test, P <0.05 for the difference was statistically significant. Results After treatment, heart rate (HR), respiratory rate (RR), mean arterial pressure (MAP), oxyhemoglobin saturation (Sa O2), arterial oxygen Arterial oxygen pressure (Pa O2), arterial carbon dioxide pressure (Pa CO2), systolic blood pressure (SBP) and diastolic blood pressure (DBP), the differences were statistically significant Significance (all P <0.05). The indexes in the observation group before and after treatment were significantly different (all P <0.05), and there were significant differences in HR, RR, MAP, Sa O2, Pa O2 and SBP in the control group after treatment (All P <0.05). The total effective rate in observation group was 88.24%, which was significantly higher than that in control group (67.65%), the difference was statistically significant (P <0.05). The stability of the observation group was 94.12% (32/34), while that of the control group was 70.59% (24/34). The difference was statistically significant (P <0.05). Conclusion Noninvasive ventilator can effectively improve the vital signs and physical status of patients with acute left heart failure, the effect is significant and worthy of clinical promotion and application.
