目的:观察并探讨血必净注射液与血液灌流(hemoperfusion HP)联用治疗急性百草枯(Paraquat,PQ)中毒患者的临床效果。方法:选择自2011年4月—2015年5月在医院接受治疗的急性PQ中毒患者52例,在常规PQ中毒急救处理的基础上分为灌流组(对照组26例)和灌流联合血必净治疗组(观察组26例)。统计并比较两组治疗前后动脉血气指标(PaO_2和PaCO_2)、脏器损伤血清学指标,包括谷丙转氨酶(ALT)、谷草转氨酶(AST)、肌酸激酶(CK)、肌酸激酶同工酶(CK-MB)水平和肌酐(Cr)及预后情况。结果:对照组死亡16例,治愈10例,治愈率38.4%;观察组死亡8例,治愈18例,治愈率69.2%,观察组死亡率显著低于对照组,两组相比差异具有统计学意义(P<0.05);两组患者治疗后ALT、AST、CK、CK-MB和Cr明显下降,观察组显著低于对照组,差异具有统计学意义(P<0.05)。结论:血必净注射液与HP联用疗法可以保护PQ中毒患者的内脏正常功能,减少多脏器功能障碍发生,有效提高治愈率。
Objective: To observe and discuss the clinical effect of Xuebijing injection combined with hemoperfusion HP in the treatment of acute paraquat poisoning. Methods: From April 2011 to May 2015, 52 patients with acute PQ poisoning who were treated in the hospital were divided into two groups: perfused group (26 cases in control group) and perfused with Xuebijing Treatment group (observation group of 26 cases). The arterial blood gas indexes (PaO 2 and PaCO 2) and organ injury serological indexes including the levels of ALT, AST, creatine kinase (CK), creatine kinase isoenzyme (CK-MB) levels and creatinine (Cr) and prognosis. Results: In the control group, 16 patients died and 10 patients were cured. The cure rate was 38.4%. In the observation group, 8 patients died and 18 patients were cured. The cure rate was 69.2%. The mortality in the observation group was significantly lower than that in the control group. (P <0.05). The levels of ALT, AST, CK, CK-MB and Cr in the two groups were significantly decreased after treatment, the observation group was significantly lower than the control group, the difference was statistically significant (P <0.05). Conclusion: Xuebijing Injection combined with HP can protect the normal function of visceral organs in patients with PQ poisoning, reduce the occurrence of multiple organ dysfunction and effectively improve the cure rate.
