[目的]探讨老年功能性消化不良(FD)患者心率变异率(HRV)的改变及其临床意义。[方法]选取我院2011年9月~2013年9月60例老年FD患者,32例老年消化性溃疡(PU)患者,另选38例体检健康者作为对照组,对所有患者及体检健康者行24动态心电图(HRV)检测,将各组心率变异率的时域指标与频率指标进行统计学分析。[结果]FD组SDNN、rMSSD、PNN50、HF均低于对照组,差异有统计学意义(P<0.05),SDANN、ULF、LF、LF/HF高于对照组,差异有统计学意义(P<0.05)。PU组与对照组相比,SDNN、rMSSD、PNN50、HF均高于对照组,差异有统计学意义(P<0.05),LF/HF则小于对照组,差异有统计学意义(P<0.05),SDANN、ULF、LF与对照组相比,差异无统计学意义。FD组与PU组相比较,SDNN、rMSSD、PNN50、HF均低于PU组,差异有统计学意义(P<0.05),其余指标则高于PU组,差异有统计学意义(P<0.05)。[结论]老年FD患者的心率变异性有显著地改变,具体表现为交感神经兴奋性增强,而迷走神经兴奋性下降。分析患者的心率变异性对于老年功能性患者的诊断与治疗具有很大的临床参考价值。
[Objective] To investigate the changes of heart rate variability (HRV) in elderly patients with functional dyspepsia (FD) and its clinical significance. [Methods] Sixty elderly FD patients and 32 elderly patients with peptic ulcer (PU) were selected from September 2011 to September 2013 in our hospital. Another 38 healthy subjects were selected as control group. All patients and healthy subjects Line 24 dynamic electrocardiogram (HRV) detection, the rate of each group of heart rate variability in time domain and frequency indicators for statistical analysis. [Results] The levels of SDNN, rMSSD, PNN50 and HF in FD group were significantly lower than those in control group (P <0.05), while the levels of SDANN, ULF, LF and HF in HF group were significantly higher than those in control group <0.05). Compared with the control group, the levels of SDNN, rMSSD, PNN50 and HF in PU group were significantly higher than those in control group (P <0.05), and LF / HF was smaller than that in control group (P <0.05) , SDANN, ULF, LF compared with the control group, the difference was not statistically significant. Compared with PU group, the levels of SDNN, rMSSD, PNN50 and HF in FD group were lower than that in PU group, the difference was statistically significant (P <0.05), and the other indexes were higher than that in PU group (P <0.05) . [Conclusion] The heart rate variability of elderly patients with FD have significantly changed, which shows the enhancement of sympathetic excitement and the decrease of vagal excitability. Analysis of the patient’s heart rate variability for the elderly patients with functional diagnosis and treatment has great clinical value.
