各省、自治区、直辖市卫生厅（局）：根据《卫生部健康相关产品审批工作程序》，现印发卫生部健康相关产品卫生许可申请表（式样），申请表自印发之日起启用。１９９９年９月１日前已通过省级卫生行政部门初审的产品，仍可使用原申请表向卫生部审评机构申报。特此通知。附件：
Health Departments (Bureaus) of provinces, autonomous regions, and municipalities directly under the Central Government: According to the “Health and Related Products Examination and Approval Procedures of the Ministry of Health,” the Ministry of Health is now issuing health-related product hygiene permit application forms (patterns), and the application form will be activated from the date of issuance. Products that have passed preliminary examination by provincial health administrative departments before September 1, 1999 can still be reported to the evaluation agency of the Ministry of Health using the original application form.
