单核吞噬细胞系统对肾小球既有免疫保护作用,又有免疫损伤作用。前者表现在单核吞噬细胞系统功能减低与原发性肾小球肾炎发病有关,增强能防治原发性肾小球肾炎;后者表现在单核-巨噬细胞在肾小球聚积可致其损伤。前者的机制是由于单核吞噬细胞系统能清除循环免疫复合物和肾小球内的免疫复合物,后者的机制是由于单核-巨噬细胞为炎症效应细胞。单核-巨噬细胞聚积是由于肾小球内免疫复合物过多,故防治原发性肾小球肾炎应是增强单核吞噬细胞系统功能。为抑制免疫损伤的副作用,可适当配用激素等药物。
