患者女性,70岁,住院号57910。因右季肋部针刺样疼痛伴发烧20天,抗炎治疗效果不佳,入院。查体体温39℃,重病容,衰竭明显。右季肋部轻度水肿,6至8肋间明显压痛、叩击痛。肝大,下缘在锁骨中线肋下4cm,中等硬,明显触痛。脾未触及。化验白血球总数78,000/mm~3,有中毒颗粒,N93％。碱性磷酸酶9.3U。胎甲球阴性。肝功能正常。总蛋白7.5g％,白蛋白3.0g％,球蛋白
