Substance P (SP) is a neuropeptide that is associated with mast cells. Neuropeptide-containing nerves exert the regulatory mechanisms by which the nervous system responds to host defense. The authors report the identification of SP and mast cells in the temporomandibular joint (TMJ). Cases and Methods All 9 patients with TMJ dysfunction were female. The average age was 38 years old (32-40 years). Unilateral TMJ dysfunction was found in 4 cases (44%) and bilateral (5 cases) in 56 cases. All cases were dislocation of the articular disc, most of them with varying degrees of TMJ degeneration, 8 cases were previously detected by polymerase chain reaction (PCR)
