目的评估随机尿香草扁桃酸与肌酐比值(VMA/Cr)对神经母细胞瘤诊断的价值。方法通过对MEDLINE、EMbase、中国期刊全文数据库、中国科技期刊全文数据库、万方数据库进行系统检索,检索起止时间为建库之日至2014年7月收录的所有已正式发表的科研文献,英文检索词包括:vanilmandelic acid,creatinine,VMA,neuroblastoma,NB;中文检索词包括:“香草扁桃酸”或“VMA”、“肌酐”、“神经母细胞瘤”或“NB”。结果神经母细胞瘤组与健康对照组随机VMA/Cr水平有统计学意义,神经母细胞瘤组随机尿VMA/Cr水平和阳性率显著高于健康对照组。随机尿VMA/Cr用于诊断神经母细胞瘤的汇总敏感度为0.84(95%CI:0.80~0.88),汇总特异度为0.93(95%CI:0.92~0.95),SROC AUC为0.9458。结论随机VMA/Cr用于诊断神经母细胞瘤的敏感度和特异度都较高,具有较高的诊断应用价值。
Objective To assess the value of random urinary vanillic mandelate to creatinine ratio (VMA / Cr) in the diagnosis of neuroblastoma. Methods All the published research articles were collected from MEDLINE, EMbase, Chinese Journal Full-text Database, Chinese Sci-tech Periodical Full-text Database and Wanfang Database, and searched from start to finish date to July 2014. Words include: vanilmandelic acid, creatinine, VMA, neuroblastoma, NB; Chinese terms include: “vanillyl mandelic acid” or “VMA”, “creatinine”, “neuroblastoma” or “ NB ”. Results The levels of VMA / Cr in neuroblastoma group and healthy control group were statistically significant. The levels of VMA / Cr and the positive rate of random urine in neuroblastoma group were significantly higher than those in healthy control group. The pooled sensitivities of random urine VMA / Cr for the diagnosis of neuroblastoma were 0.84 (95% CI: 0.80-0.88), a pooled specificity of 0.93 (95% CI: 0.92-0.95) and a SROC AUC of 0.9458. Conclusions The sensitivity and specificity of random VMA / Cr for the diagnosis of neuroblastoma are high and have high diagnostic value.
